The MGBS Planning Form
"Our
guide is more of a suggested outline for planning, rather than a form to
complete, and stick away in a drawer.
Planning one’s final rites can be one of self-reflection and
exploration. Some people find forms confining,
and we in no way want to dictate how anyone should feel about their plans. Planning one’s final event can be
empowering. Some people who have
planned, say they feel freer to live knowing that their wishes are known.
We
suggest you keep your plans on a computer, and print out a copy of them and
give them to the person or people you think need to know what you want, and
will be the person or people most likely to be open to what you want and
willing to come as close to your plan as possible.
We have divided the process into four sections: What, Where, Who and How. Feel free to develop your plan as you see fit." - Intro excerpt from the Midwest Green Burial Society Planning Form
